Home refactored with modular components (SearchHeader, HomeCta, FrequentDiseases, SeasonAlert, PracticalGuides).
Replaced hardcoded colors with theme tokens in SeasonAlert and HomeCta.
Updated navigation, i18n, and CLAUDE.md.
